93 was retired effective 10/16/2003 and should not appear on new remittances.
Official retirement note: As of 004010, CAS at the claim level is optional.
If 93 appears on an old ERA you are reworking, interpret it per the remittance date, then follow the current code's guidance for any corrected claim.
