A2 Denial Code (Deactivated) – Use 45

A2 is a retired claim adjustment reason code. Payers stopped using it on 01/01/2008. Current remittances use 45 instead.

Seeing A2 on a Remittance?

A2 was retired effective 01/01/2008 and should not appear on new remittances.
Payers now report this situation with 45 — check that page for current guidance.
Official retirement note: Use Code 45 with Group Code 'CO' or use another appropriate specific adjustment code.
If A2 appears on an old ERA you are reworking, interpret it per the remittance date, then follow the current code's guidance for any corrected claim.
